Participant contract and parent consent form (see Forms page)

Physical Fitness & Medical History form (see Forms page)

Copy of birth certificate (new participants only)

Two copies of 2008 - 2009 complete report card (final available in June '09)

Current wallet sized photo (no hats or sunglasses)

Registration fee of $275 football, $225 cheer

Deposit of $100 (may be refunded if requirements are met)
